The Key Distinction

In anxiety disorders, anxiety itself is the primary symptom. You worry excessively about various life concerns—health, finances, relationships—and experience physical symptoms of stress.

In OCD, anxiety is secondary—it results from intrusive obsessive thoughts. The anxiety drives compulsive behaviors that temporarily relieve the distress but ultimately reinforce the cycle.

OCD Characteristics

  • Intrusive, unwanted, often bizarre or disturbing thoughts
  • Compulsive behaviors or mental rituals to neutralize anxiety
  • Recognition that obsessions are irrational (usually)
  • Significant time spent on obsessions/compulsions (often 1+ hours daily)

Anxiety Disorder Characteristics

  • Excessive worry about realistic life concerns
  • Physical symptoms: tension, restlessness, sleep problems
  • No compulsive behaviors to relieve anxiety
  • Avoidance of anxiety-provoking situations

Can You Have Both?

Absolutely. About 75% of people with OCD also have an anxiety disorder. Having both can intensify symptoms and complicate treatment, which is why accurate diagnosis is essential.

Treatment Differences

While both conditions may be treated with SSRIs, OCD requires higher doses. OCD-specific treatment includes Exposure and Response Prevention (ERP) therapy. Standard anxiety treatments like relaxation techniques may not help—and can sometimes worsen—OCD symptoms.
